To qualify for ODSP, you need to:
- Be 18 years or older.
- Live in Ontario.
- Have a significant physical or mental disability expected to last a year or more, affecting your ability to work and care for yourself.
Once you're approved for ODSP benefits, you'll get a caseworker. Ask your caseworker for the Mandatory Special Necessities Benefit Form (MSN form).
Make an appointment with your family doctor or health practitioner. Talk to them about your needs and have them fill out the MSN form.
Check what's approved on the MSN form, including quantities. Submit the form to your caseworker. Your caseworker will then ask you to get a quote for your supplies.
Reach out to Healthwick to get a quote for your monthly supplies. Have the following info ready:
- Personal Info: Your name, address, phone number, and email.
- Caseworker Info: Their name, contact details, and ODSP office location.
- MID Number: Your ODSP Member ID.
- Product Details: The specific products you need (brand, model, size) and the monthly supply amount.
- Form and Quote Details: Confirm if your doctor has filled out the MSN form and if your caseworker asked for multiple vendor quotes.
- Shipping Preferences: Do you need discreet shipping? Do you want automatic monthly orders or prefer to contact Healthwick as needed?
Important Note: Make sure all this information matches what's on the MSN form.
Once Healthwick hears back from your caseworker (this can take up to 15 business days), they'll contact you about your approval. After you're approved, they can place your first ODSP order.
