Healthwick is pleased to offer Direct Billing for most major private and public insurance groups in Canada. As every plan is very individual and has different coverage, be sure to review your coverage with your insurance agent or caseworker before contacting us, so that we can make the direct billing process as quick and easy as possible.
Here are the most common insurance groups and the required information for us to help you with direct billing each:
Ontario Disability Support Program (ODSP) - Click here for full information
|You need a completed Mandatory Special Necessities Benefit Form, as completed by your doctor or nurse, plus your case worker’s direct phone line. We need to get prior approval from your case worker over the phone before we will direct bill; sometimes this takes time if they are away from their desk and we need to leave a message.|
Third Party Insurance Billing
|It is possible that you have coverage through your third party insurance program such as Great-West Life, Manulife, Sunlife, etc. Third party insurance billings are evaluated on an individual basis; the protocol to bill these insurance companies is different and is on a case-by-case evaluation. In some cases we can do direct billing, and if not you will have the option to pay upfront and then get reimbursed by your personal insurance plan. Most private insurance companies require you to have a doctor’s prescription with diagnosis, a receipt and a claim form designed by the company (usually available to download online).|
Blue Cross Veteran's Affairs
|You will need to provide Healthwick with your ‘K’ number and a doctor’s prescription with diagnosis. We need to get prior approval from Blue Cross over the phone before we will direct bill them as only certain items are covered.|
